WAIVER

By registering for and participating in any camp, clinic, practice, training session, competition, or activity conducted by Northwoods Throws Club LLC ("Northwoods Throws Club"), the participant and/or parent or legal guardian of a minor participant acknowledges and agrees to the following:

Assumption of Risk

Participation in track and field throwing events, strength and conditioning activities, drills, training exercises, and related athletic activities involves inherent risks, including but not limited to serious bodily injury, illness, permanent disability, death, and property damage. These risks may result from the participant's own actions, the actions of others, equipment, facilities, weather conditions, or other known and unknown causes.

The participant and/or parent or legal guardian voluntarily assumes all risks associated with participation in Northwoods Throws Club activities.

Release of Liability

To the fullest extent permitted by law, the participant and/or parent or legal guardian releases, waives, discharges, and agrees not to bring any claim against:

• Northwoods Throws Club LLC;

• Its owners, members, officers, directors, employees, volunteers, agents, and coaches;

• Howard-Suamico School District;

• Any facility owners, operators, sponsors, or affiliated organizations utilized for Club activities;

for any injury, illness, death, loss, damage, expense, or liability arising out of or related to participation in Northwoods Throws Club activities, including claims resulting from ordinary negligence.

Medical Treatment Authorization

The participant and/or parent or legal guardian authorizes Northwoods Throws Club staff, coaches, volunteers, and emergency personnel to secure emergency medical treatment if deemed necessary. The participant and/or parent or legal guardian accepts full responsibility for any resulting medical expenses.

Physical Fitness

The participant and/or parent or legal guardian certifies that the participant is physically able to participate in athletic activities and has no known medical condition that would make participation unsafe.

Rules and Conduct

The participant agrees to follow all safety rules, instructions, and directions provided by coaches, staff, and facility personnel. Northwoods Throws Club reserves the right to remove any participant whose conduct creates a safety concern.

Electronic Consent

By checking the box on the registration form indicating agreement to these terms and completing registration, the participant and/or parent or legal guardian acknowledges that they have read, understood, and voluntarily accepted this waiver. The registration form will be available at checkout.
